Admission Requirements

Admission from Hospital:

FL2 (with approval number is applicable) and PASARR number
MAR – Medication Administration Record
IV Administration Record (past 14 days)
History and Physical
Chest X-Ray or PPD 6. Lab Reports if applicable
Therapy Notes if Applicable
Signed Discharge Summary or
Transfer form with orders; signed by Physician.

Admission from Home:

FL2 (with approval number is applicable) and PASARR number
History and Physical
Signed Doctors Orders
Chest X-Ray or PPD
Home Health Notes if applicable
If admission is within two weeks of a hospital discharge and under Medicare Part A, requirements will follow under “Admission from Hospital”

Admission from another Nursing Facility:

FL2 (with approval number is applicable) and PASARR number
History and Physical
Chest X-Ray or PPD
Lab Reports if applicable
Transfer form with orders; signed by Physician
If transferring under Medicare Part A, requirements will follow under “Admission from Hospital”
